An Integrated Solution for Sustainable Care for Multimorbid
Elderly Patients with Dementia

That is based on ICT solution for optimizing clinical practice in the treatment and management of multimorbid patients with Mild Cognitive Impairment or Mild Dementia

Project

CAREPATH is a Horizon 2020’s project that proposes an ICT based solution for the optimization of clinical practice in the treatment and management of multimorbid patients with Mild Cognitive Impairment or Mild Dementia. CAREPATH solution will target a more complex multimorbidity scenario and follows an integrated patient-centered approach, in order to develop a flexible and modular system to deliver a best care, adapted framework for increasing the independence and Quality of Life (QoL) of multimorbid patients with dementia.

Vision

Follow a patient-centred integrated care approach, in order to develop a flexible and modular system that will provide improved interventions for the management of multimorbid elderly patients with dementia.

Scope

CAREPATH propose to follow a patient-centered integrated care approach to develop a flexible and modular system that will provide improved interventions for the management of multimorbid elderly patient with dementia.

Objectives

Patient-centred approach

Through an integrated ICT platform that jointly and efficiently addresses multimorbidity and dementia challenges while also considering the intrinsic capacity of the patient

Technical Validation and Usability (TVU) study

To evaluate usability, UX (user experience), and usefulness of the CAREPATH platform. Safety issues will also be addressed.

Propose a technology-driven framework

Enabling patient-focussed Integrated Care pathways, with associated healthcare models for the management of multimorbid elderly patients with dementia for QoL improvement

Develop Quality Key Performance Indicators

For the management of multimorbid elderly patient with dementia exploiting the findings of the CAREPATH two-year CI

Health Economics impact assessment

In terms of cost effectiveness and care provision inequalities

Ambitions

Integrated personalized delivery of health and social care

Improvement of quality of life for the patient and his/her informal caregiver

Improvement of quality of life for the patient and his/her informal caregiver

CAREPATH will take advantage of the positive outcomes from existing models and from experiences previously piloted at different sites to achieve the objective of improvement of QoL of patient and close environment.

Improvement of working conditions of health- and social care providers involved in the supply chain

CAREPATH is built from the very beginning taking into account the professionals needs. the expected results will incur in more specialisation for their work.

Develop a system that conforms to European Medical Device Regulation

The CAREPATH system is intended to be used for diagnostic and therapeutic purposes in the treatment of patients with MCI and mild dementia also being affected by multimorbidity.
